Risk Management General Liability maintains:

We assess any and all potential risks within the District to establish whether a risk is present and, if so, the range or magnitude of that risk.

Do you need to report an accident or file a claim?

Below are the forms and/or links you will need to report accidents and property damage or file a claim. The district has a new Third-Party Administrator, USIS, for handling property and liability claims. USIS replaced Sedgwick Delegated Authority (formerly CJW) effective July 1, 2021.

School Insurance of Florida is the provider of group accident programs, such as interscholastic, extended day, and technical colleges.

  • Student Accident/Injury Report

    Please use this form to submit student accidents/injuries electronically. Handwritten reports will no longer be accepted.

    **If entering multiple incident reports, you must click the RESET button after each submission. 

    Q. When should Risk Management be notified?
    A. In accordance with School Board Policy JLCE, student incidents are required to be reported within 48 hours of occurrence.

    Q. What type of incident or injury should be reported to Risk Management?
    A. Risk Mgmt. does not need to know about every bump, bruise, headache or vomit that comes to the clinic.

    Here are 3 basic scenarios when an incident should be reported to Risk Management:

    • The incident or injury warrants immediate medical attention by either parent pick up, school takes them, or 911 is called. This includes assault.
    • The student received medical treatment for school incident resulting in doctor’s note provided excusing absence, limiting activity (no PE, on crutches, etc.)
    • Parent is inquiring about reimbursement of medical expenses due to incident or involved an attorney.

     

    Student Athletes Incidents:
    OCPS has partnered with School Insurance of Florida to provide accident insurance for student athletes while participating in FHSAA sanctioned sports.

    • Extended Day Services 
  • Workers' Compensation

    Any employee who is injured on the job is required to immediately report the injury to the appropriate supervisor and complete the required forms. Employee injuries MUST be reported to the Workers' Compensation department.

  • General Incident Report

    In accordance with School Board Policy KI, visitor incidents are required to be reported within 48 hours of occurrence. 

    Use this fillable form to report all other incidents. Not intended for Student or Employee injury

  • When an employee is involved in a crash/incident while driving a District owned/leased vehicle (school bus or white fleet) the two forms below must be completed:

  • Prop 5 – LAPTOPS / DIGITAL DEVICES
    ONLY use this form to report damages, theft, or failure to return of laptops or digital devices assigned to students or employees.

    • A police report is required, if the loss/damage is a result of break-in or strong-armed robbery.


    Prop 5 – ALL OTHER ASSETS
    This form is used to report damages or theft of OCPS assets, Inventory Reporting, or Hurricane Damage. This includes, but is not limited to, damage to buildings, furniture, fixtures, and other equipment. 
    This form is NOT intended to be used for Personal Property that is lost or damaged.

    • A $750 deductible applies for all covered losses. 
    • A police report is required, if the loss/damage is a result of theft or vandalism.
